However to join the waiting list please complete the form below.\nLondon Leadership Academy are delighted to be working with NAPC to deliver this online learning session on population health management and managing health inequalities. \nThe session\, facilitated by Dr Nav Chana\, Annabel Beasley and Andy Mullins from NAPC\, will be providing a practical insight into population health management and managing health inequalities.  By the end of this webinar\, participants should: \n-Be reminded of the social determinants of health and wellbeing \n-Have a base level understanding of population health management – an increasingly important concept behind new care model development \n-Understand how population health management is key to tackling health inequalities \nNational Association of Primary Care (NAPC) is a not-for-profit\, social purpose development and support organisation whose mission is to improve the health and wellbeing of defined populations locally and nationally. \nAnnabel Beasley: Annabel has worked for a large biomedical research charity and the academic sector. For the past 11 years she has been working in the health sector and has a background in workforce\, education\, leadership and developing collaborative networks. \nDr Nav Chana: Nav is a GP in SW London and was instrumental in developing the Primary Care Home model which later became part of NHS national policy as PCNs. He advised NHSE on workforce transformation and developed the concept of Community Education Provider Networks (now Training Hubs) at HEE. Nav is a Non-Executive Director at Kingston Hospital. \nAndy Mullins: Andy has more than 25 years of experience facilitating change across both public\, and private sectors.   His focus for the last 18 years\, has been on the health sector\, but also has experience of working with charities\, public transport\, manufacturing\, and the oil industry. \nFully booked.
